You are about to apply for the admission test to the English-taught Medicine and Surgery degree program.
In order to ensure that you complete the application process successfully, please read the following very
carefully.
A. Personal Data 1: (fill in this section accurately)
a. Under nationality/citizenship indicate the country issuing your passport, which may not correspond to
your country of birth, nor current country of residence.
b. Nationality/citizenship 2: answer this question if you have double citizenship
B. Personal Data 2:
If you have a current and valid permit of stay in Italy, please upload a copy of your permit of stay (only
required for non-E.U. citizens).
C. Application Type:
a. Under “Program Type” make sure you select the “Degree-seeking” option in the drop down menu.
b. Target level of studies: select undergraduate
D. Degree-seeking section:
a. Duration of your program: select 6 Year Medicine and Surgery
b. School/Faculty, first choice: select Medicine and Surgery – Rome
c. Program name, first choice: select Medicine and Surgery (English)
c. UCSC International Scholarship (only applicable to international students whose family income is
generated outside the EU and associated countries): if you wish to be automatically considered for the
UCSC International Scholarship, ensure you check “yes”. Students who obtain the UCSC International
Scholarship may not apply for the scholarships offered by EDUcatt.
d. Transfer students: check “no”. We are currently not accepting transfer requests.
E. Academic Information:
a. Education History: indicate how many years of formal schooling you’ve completed before accessing
university level, e.g. Italy has 13 years from beginning of primary to end of High School, whereas other
countries may have less, 11 or 12 years.
b. Complete High School 1 with the information regarding the most recent institution you’ve attended, and
where you have completed or are in the process of completing your studies.

c. Make sure you upload a copy of your diploma, if available. If it is not available yet, you can leave this
blank.
F. Additional Academic Info:
b. You must provide any English Proficiency test result. In the case of TOEFL, please provide the test Centre
with the UCSC International Toefl code (2605) and request a copy to be sent to our office (address below).
If you have not yet obtained a certificate, please provide the scheduled test date. If you are exempted from
providing an English Proficiency test according to Cattolica admission policy, please insert “N/A” in the test
date section.
G. Additional Information:
Agent: if applicable, make sure that you indicate the name, address and contact information of the agent or
agency that has assisted you with the application process, or that is working with you in completing the
admission’s process to UCSC.
H. Additional documents upload: upload a copy of your passport (only the page with your personal details
on) or ID card in Latin alphabet.
I. Application Essay: Provide in at least a couple of paragraphs your primary motivations to study with us.
The essay should be written in the language in which your chosen program is taught.

At this stage, documents do not require Italian translation if they are issued in English, French, Portuguese
or Spanish. The documents have to be uploaded in a legible format. If their size exceeds the limit allowed
by the online application (2400 KB), please send them via e-mail to [email protected] and
upload a Word file instead where you have indicated “document sent via e-mail”.
J. Submission of Application: upon submission of your application you will receive an email acknowledging
the successful process
