Qualsiasi fatto materiale deve essere rivelato agli Assicuratori. Un fatto materiale viene considerato tale quando
seguendo la prudente tecnica assuntiva/assicurativa un assicuratore verrebbe influenzato nella valutazione ed
accettazione del rischio. Omissione o falsa dichiarazione possono costituire ragione di annullamento del contratto. In
caso di qualsivoglia dubbio se gli Assicuratori debbano essere o meno informati di un fatto materiale, per Vostra tutela Vi
suggeriamo di citarlo in ogni caso.
Tutti i dati personali da Voi forniti nel compilare la presente proposta verranno trattati in via riservata dalle societá del
gruppo (assicurativo) Royal & Sun Alliance e non saranno divulgati a terzi se non in caso di autorizzazione da parte
Vostra o nei casi consentiti dalla Legge.
Entrambe le parti del contratto hanno la facoltá di scegliere la Legge applicabile al contratto. A meno che non sia stato
concordato, qualunque disputa sull’interpretazione di questa Proposta o del contratto che si basa sulla Proposta sará
governata dalla Legge italiana e sará risolta sotto la giurisdizione delle corti della Repubblica Italiana.
Consigliamo di mantenere traccia di tutte le informazioni fornite agli Assicuratori per la valutazione del rischio.
Per quanto di mia conoscenza dichiaro che le informazioni riportate sono complete e veritiere in ogni aspetto. Sono
altresí consapevole che l’Assicuratore determinerá le condizioni ed i termini sulla base delle informazioni contenute in
questa proposta.
Dichiaro inoltre che se la presente Proposta é stata compilata da un’altra persona diversa dal sottoscritto, tale persona é
stata autorizzata ad agire in nome e per conto del proponente ai fini della stipulazione di una polizza assicurativa.
All material facts must be disclosed to Underwriters. A material fact is one which a prudent Underwriter would regard as likely to influence the acceptance or
assessment of the risk. Non-disclosure or misrepresentation of a material fact may result in the insurance being void. If you are in any doubt whether a fact
should be considered material, you should disclose this fact.
All personal information supplied by you will be treated in confidence by the Royal & Sun Alliance Insurance Group of companies and will not be disclosed to any
third parties except where your consent has been received or where permitted by law.
The parties to the Policy have the right to choose the law applicable to the Policy.
Unless the parties agree otherwise in writing any dispute concerning the interpretation of this Proposal or the Policy shall be governed and construed in
accordance with English Law and shall be resolved within the exclusive jurisdiction of the courts of England and Wales.
You should also keep your own record of all information supplied to us in arranging this insurance.
I declare that the particulars and answers are correct and complete in every respect to my knowledge and believe. I agree that the Proposal Form shall form the
basis of the contract of insurance between me and Underwriters.
I further declare and agree that if the statement and particulars above have been completed by another person other than the undersigned such person is
deemed to act on behalf of the proposer for the purpose of completion purposes.
