1. Dichiaro di aver preso i necessari accordi affinché il
minore sia accompagnato alla partenza e preso in
custodia allo scalo intermedio e all’arrivo dalle persone
nominate. Tali persone rimarranno all’aeroporto finché
l’aereo non abbia decollato e saranno presenti
all’aeroporto per l’ora prevista di arrivo del volo
2. Qualora il minore non sia preso in custodia come indicato,
io sottoscritto autorizzo Neos spa e gli altri Vettori
interessati a prendere qualsiasi provvedimento si renda
necessario per la custodia del minore, compreso il ritorno
all’aeroporto di partenza e accetto di indennizzare e
rimborsare Neos spa e/o gli altri Vettori delle spese da
loro sostenute nel prendere tali provvedimenti
3. Confermo che il minore è in possesso di tutti i documenti
di viaggio (passaporto, visto, certificato sanitario, etc)
richiesti dalle leggi locali
4. Io sottoscritto, in qualità di genitore o tutore, richiedo il
trasporto di tale minore non accompagnato ed attesto la
validità delle informazioni fornite.
Declaration of parents or guardian
1.
I confirm that I have provided for the minor to be
accompanied to the airport on departure and to be met
at the stopover point and on arrival by the person
named. These persons will remain at the airport until
the flight has departed and be available at the airport
at the scheduled time of arrival of the flight
2. Should the minor not be met as stated, I authorize Neos
spa and the carrier(s) concerned to take whatever
action they deem necessary for the minor’s safe
custody, including returni to the airport of departure
and I agree to indemnify and reimburse Neos spa
and/or other carrier(s) the cost and expenses incurred
by them is taking such actions
3. I certify that the minor is in possession of all travel
documents (passport,visa,health certificate, etc) required
by local laws
4. I, the undersigned, as parent or guardian, request the
carriage of the unaccompanied minor and certify that
the information provided is accurate
