TASIS SHUTTLE SERVICE PERMISSION AND RELEASE OF LIABILITY FORM
I/We request that the following child(ren) enrolled at TASIS use the TASIS Shuttle Service,
and I/we agree to release and discharge the school and its agents and employees exercising
reasonable care, from all liability growing out of any personal injuries and/or property damage
resulting from or occurring during transit to and from school.

I/We have received a copy of the TASIS Shuttle Service Safety and Conduct Rules. My
children and I have read them, understood them and discussed them. Our child(ren) agree to
abide by these rules. I/We further understand that the failure of my child(ren) to follow these
rules may result in their not being permitted to ride the TASIS Shuttle.
Only for Middle and High School students:
I hereby give my permission for my child____________________________ to wait at the
TASIS Shuttle stop in the morning unattended and/or to be left at the TASIS Shuttle stop in
the afternoon without a parent, guardian or other adult present.

TASIS Shuttle Service Safety and Conduct Rules
1. Arrive at the TASIS Shuttle stop at least five (5) minutes before pick-up and drop-off
time. The shuttle cannot wait for tardy students.
2. Line up in single file to enter the shuttle at the TASIS Shuttle stop. Wait for the driver’s
instructions to board the shuttle.
3. Upon entering the shuttle sit down immediately and fasten your seat belt. Drivers
cannot stop the bus to ask the students to fasten the belt again.
4. Place all books and belongings where they will not slide or fall.
5. Once seated, stay in your seat until the shuttle comes to a complete stop at your TASIS
Shuttle stop, or at school.
6. Behave on the shuttle as if you were in a classroom – talk quietly and treat your
schoolmates with respect.
7. Please wait until you get off the shuttle to eat or drink.
8. Keep arms, legs, and any carry-on items out of the aisle. It is important that the aisle be
kept clear at all times.
9. Open windows only with permission of the driver. Never throw anything out of the
windows, or put your hands, arms or head out of the windows.
10. Listen to the driver and follow all instructions promptly.
PLEASE REMEMBER TO:
• speak quietly.
• keep your hand, head, or any other body part in a safe position.
• use appropriate language (if it is not acceptable while on school grounds, it is not
acceptable on the TASIS shuttle).
• be nice to other students. Avoid fighting and pushing.
• stay seated.
• wear your seat belt at all times.
• avoid using pens, pencils or any other sharp object.
FAILURE TO FOLLOW ANY OF THESE RULES, AS WELL AS TARDY PICK-UPS FROM
THE TASIS SHUTTLE STOP AT THE END OF THE DAY, AFTER THREE TIMES WILL
RESULT IN THE STUDENT NOT BEING ALLOWED TO USE THE TASIS SHUTTLE
SERVICE.

The TASIS Shuttle may be 3 to 5 minutes early or late, depending on traffic and on the
weather conditions (snow).
Elementary School Students are not allowed to disembark from the TASIS Shuttle in
the afternoon without a parent, guardian or other adult present. In this case the student
will remain on the shuttle, and be brought back to school once the TASIS Shuttle has
